Our Community Services are here to help you stay healthy and safe in your own home.
We provide services across South Tyneside, Gateshead and Sunderland. We work with people of all ages, from babies to older adults. We get you the help you need, closer to where you live, outside of our hospitals.
Our teams include nurses, therapists, and other health professionals. They visit you at home, in schools, or in local clinics. Click here for full details on where our sites are.
We also help with things like managing long-term health conditions, recovering after illness and helping you stay independent.
