Our District Nursing team cares for people who cannot leave their home. Our nurses visit you at home to give the care you need.

We give planned care based on your health needs. This care may be asked for by your GP, hospital or another service. We will check your needs and make a care plan with you.

We want to help you stay as independent as possible. We also support you to care for yourself when you can.

  • Wound care (like pressure sores or leg ulcers)
  • Help after surgery (like dressing changes or removing stitches)
  • Bladder care (like catheter changes)
  • Help with bowel care
  • Help with diabetes and insulin
  • Checking blood pressure, pulse and temperature
  • Taking samples (like blood or urine)
  • Giving medicines (like eye drops or injections)

Our District Nursing Team is made up of a range of skilled professionals who work together to provide care at home and in the community. This includes:

  • Specialist District Nurses 
  • Community Nurses
  • Healthcare Assistants 

We care for people who are housebound. This means you cannot leave your home to go to a clinic, even with help. You may be housebound for a short time or for a long time. If you can travel to a clinic, you are not classed as housebound. 

You must be registered with a GP in South Tyneside or Sunderland. We do not take self-referrals. You must be referred by a healthcare worker. 

We will look at your referral and decide how we can help. A Specialist District Nurse will lead your care. You might see different team members depending on what care you need.

Visit times may change. We see people who need urgent care first. You may also see a student nurse during your care. We will check your care often and stop visits when you no longer need them.

We accept referrals for people who are housebound and registered with a GP in South Tyneside or Sunderland.
