Our Hospital at Home services gives you the care you need at home instead of in the hospital. This care is safe and easy.

It means you can have your treatment at home, or leave hospital sooner and continue your care where you feel most comfortable.

Many people recover better at home than in hospital. The Hospital at Home team will check first to make sure it is safe for you to be cared for at home. If Hospital at Home is right for you, the team will talk with you and your doctor about your care. You will then go home to continue your treatment and recovery. If you need to stay in hospital, you will not be referred to the Hospital at Home team. 

The Hospital at Home team includes different staff members. They will work together to manage your care. This may be in person, through video calls or both.

If you have a smartphone or tablet, the team will help you download the app you need. If you don’t have one, we can lend you a device. 

The team is made up of:

  • Doctors 
  • Nurses 
  • Health Care Assistants
  • Pharmacists
  • Physiotherapist Dieticians 
  • and Occupational Therapists.

If Hospital at Home is right for you, the team will talk with you and your doctor about your care. You will then go home to continue your treatment and recovery.

We will give you:

  • a clear care plan
  • the medicine you need

Once you are home, the team will make sure everything is set up and working. We will also tell your GP that you are being cared for by Hospital at Home. The team will stay in touch to check how you are doing.
