Recovery at Home is the name for the teams that care for people who need urgent help in the community. We care for people across the whole of South Tyneside and Sunderland. We visit people in their own homes and in residential or nursing homes.
We are a team of nurses, health care assistants, physiotherapists, occupational therapists and social care professionals.
We provide care 24 hours a day, 7 days a week. We can help you at home if you or someone you care for, suddenly feels unwell. We can also help if you have an existing condition that has gotten worse and needs urgent treatment or support.
We can help you with things like:
- Breathing problems, like COPD
- Worsening symptoms of a long-term condition
- Stomach pain that is getting worse
- Water infections and blocked catheters
- A fall or trouble moving around
- Serious cuts and wounds
- A breakdown in care, i.e. if your usual carer is no longer able to support you
You must be over the age of 18. You must also be registered with a GP practice in South Tyneside or Sunderland. You most also live in South Tyneside or Sunderland. If you are under 18 or have a serious or life-threatening illness or injury, please call 111 or 999.
When you call someone will ask you a few questions about how you’re feeling. This helps them understand what’s going on. It also helps them to get you the right care quickly.
They might:
- give you advice over the phone or
- send someone to visit you at home if needed. If a visit is arranged, we’ll tell you when to expect them.
Important: Recovery at Home is not a replacement for 111 or 999. If you or someone else needs emergency help, call 999 right away.
