We provide a nursing service for children age 0-19 years.
We care for children and young people with short and long term health needs in their home.
Our job is to:
-
Help children leave hospital earlier so they can be cared for at home
-
Prevent children being admitted to hospital when they don't need to
-
Reduce the number of outpatient appointments needed
-
Act as a link between the community, hospitals and all other agencies involved with the family
-
Assess the physical, emotional and social needs of the child and family. We know everyone is different
-
Teach and support families with skills and knowledge to help them to be cared for in the community
-
Talk to children and their families about how to live a healthy lifestyle
